More than half a billion pounds to help people return home from hospital

For the vast majority of people, recovery can be quicker when care is provided within your own home, this also promotes independence and enhances well-being. From 1st September Patients who require follow on care further to being discharged from hospital, including the older communities and people with disabilities, will be supported by a £588 million fund to cover adult social care or the immediate costs of care in their own home.
 
This funding will be in place to provide up to six weeks of additional ongoing care and support to those most vulnerable and comprehensive care and health assessments will be in place to ensure that people can be discharged safely.
