As a Care Quality Commission registered supported living service, we have a legal responsibility to ensure the safety of our service users and staff. It’s important for you to understand that whilst we are a profit making, high cash flowing and recession proof business, we are a heart centered business and put the needs and wellbeing of our service users first. You do not need to have a care background in order to have a supported living business, many of our franchisees have other non care related business or are still working in a 9-5 job and desire financial and lifestyle freedom. So what does it look like being a supported living provider? We provide care and support for adults with mental health needs who are to be discharged from a mental health hospital under section 117 aftercare. Some people who have been kept in hospital under the Mental Health Act can get help and support that is funded by the NHS or social care after they leave hospital. The law that gives this right is section 117 of the Mental Health Act, and it is often referred to as ‘section 117 aftercare’. Aftercare is the help they will get in the community after they leave the hospital. We work with service users who have personality disorders, eating disorders, bi polar disorders and many more. Our role is to support them to reintegrate into the community and reduce the risk of their mental health getting worse and going back into hospital. Once we receive a referral from a social worker for someone who has a health, social and housing need. Your Registered Manager, which can be you if you are qualified and have the necessary experience. If that’s not you we will support you to recruit a Registered Manager. The Registered Manager will go to the hospital to assess the needs of the service user, their needs maybe Healthcare, social care and employment service, supported accommodation (this is what sets us apart from domiciliary services) services to meet your social, cultural and spiritual needs – as long as they meet a need that arises from or is related to their mental health condition and help reduce the risk of it getting worse.
Once the assessment is completed, the registered manager will calculate how many daily hours of support they need and send the care costs to the social worker to be agreed. Once the care costs have been agreed a hospital discharge date is set and you will work with the social worker to ensure a smooth transition into the property with the care and support that has been identified at the assessment. When the service users moves into the property, the registered manager will complete a care plan. A care plan is a document created for a person that is receiving healthcare, personal care, or other forms of support. The care plan details why a person is receiving care (their assessed health or care needs), their medical history, personal details, expected and aimed outcomes, and what care and support will be delivered to them, how, when and by whom. This ensures the standardisation of high-quality, evidence-based care that takes a holistic approach. Prosperity Housing Group aims for each service user to receive outstanding care and support with well documented evidence to support this.
